Mi Casa offers a program of intensive case management and prevention for youth and families at Lake Middle School who are at-risk for academic failure, delinquency, substance abuse, truancy, and other negative behaviors.
Mi Casa's intensive case management program relies on collaboration between the case manager, the case managed youth, the youth’s family, the school, social service agencies and law enforcement. Youth are nominated into the program by teachers or school counselors based on trouble at home or at school. Students are selected to be a part of the intensive case management program if the student and their family demonstrate a willingness to work with the case manager and the school.
Each year, the case management program at the Mi Casa Neighborhood Center maintains a caseload of 20-30 youth plus siblings and adult care givers. The case manager follows up with each youth once a week, does home visits every month and connects the families to available community resources. An important part of the program is connecting each youth with an adult mentor to help the child develop excel in school, form positive bonds and build self-esteem.
